The Mercy Difference

Independent Living Apartment
Mercy Health Partners Acute Rehab Unit is one of the few facilities offering independent living. As you toward discharge, we want you and your family to feel secure with your level of independence.

For this purpose, we have designed an apartment with a kitchen, laundry and dining. The rehabilitation team will discuss any concerns or limitations with you and your family, and provide training and equipment to overcome any barriers.

Therapeutic Pass
During the latter part of your rehabilitation, the rehabilitation team may suggest a therapeutic pass. The pass is a period of 10 hours or less during the day when you may leave the center.

Therapeutic Outings
We are one of the only rehab faclities that offer outings to patients who are ready for the experience. In the past, we have taken our patients to The Farmers’ Market, Catholic Mass, shopping, and even to a picnic on the beach.

One of the outings our patients enjoy the most is tending to our wheelchair-accessible outside garden. Here, patients are able to plant, help grow vegetables, flowers and herbs.

Your Care Team
Once admitted to the inpatient program, you and your family will work closely with the rehabilitation team, led by the physiatrist, to establish goals and develop a treatment plan. This team includes specialists from:

  • Rehabilitation nursing
  • Physical therapy
  • Occupational therapy
  • Speech-language pathology
  • Clinical social work
  • Pastoral care
  • Clinical nutrition
  • Neuropsychology
  • Case managers
  • Pharmacists
  • Respiratory Therapy

Each week, the rehabilitation team will meet with the you to discuss your goals, progress and treatment plan.

Everyone works together to provide not only expert care, but enthusiastic encouragement. Additionally, meetings with family members are held weekly with the team to review your progress, goals and discharge plans.
