What is Mercy H.E.A.R.T. Center? Mercy H.E.A.R.T. Center (Health Enhancement and Rehabilitative Therapy) is a comprehensive wellness and rehabilitation center located just west of Mercy Health Partners. Individuals are welcome to participate in several health and wellness programs provided at the facility. The team at Mercy H.E.A.R.T. Center is devoted to improving the community’s overall fitness and well being through healthy living.

If you are interested in taking an active role in improving your health, Mercy H.E.A.R.T. Center is the place to be. We work with individuals of all ages and with varying levels of need. In our Cardiac and Pulmonary Rehabilitation programs, we work with adults of all ages and backgrounds who have developed heart and lung diseases. We take into account the factors that have led up to your illness, your current physical abilities, and your short and long-term goals. We develop plans of care for each and every person we see in order to work toward achieving these goals. The ultimate goal of these programs is to help you to return to a full and productive life. Education on diet, exercise, medications, and proper medical treatment has helped many, many people to take control of their health.

In addition, we work with those who want to prevent long-term health complications. Mercy H.E.A.R.T. Center is home to a variety of wellness programs that educate on maintaining a balanced diet, and incorporate a sound exercise program along with risk factor modification. Risk factors that can put an individual at high risk for heart disease and stroke include high blood pressure, high cholesterol, tobacco abuse, physical inactivity, diabetes, stress, and overweight or obesity. When you become involved in any of our wellness programs, your goals may be focused on losing weight, feeling better and healthier, and managing any risk factors you may have.

Mercy H.E.A.R.T. Center provides a positive, supportive atmosphere for you to improve your fitness and help achieve realistic goals, regardless of your reasons!

The Team at Mercy H.E.A.R.T. Center
Mercy H.E.A.R.T. Center’s team consists of doctors, surgeons, nurse practitioners, nurses, exercise physiologists, a dietitian, and a respiratory therapist. Each team member has an important role in helping to assist you in improving your health. Depending on the program or service you are involved in, you may be working with one or a number of these individuals. The director of the Heart and Vascular Institute is LRod Hayhurst, RN, MSN. The H.E.A.R.T. Center Manager is Blair Moreau, BS.

Accreditation and Recognition
Mercy H.E.A.R.T. Center’s Cardiac and Pulmonary Rehabilitation programs are nationally accredited by the American Association of Cardiovascular and Pulmonary Rehabilitation. In addition, Mercy Health Partner’s Heart and Vascular Institute has been prestigiously recognized as a Cardiac Center of Excellence. We are also proud to have been recognized as a Priority Health Center of Excellence for our Bariatric Weight Management Program.

The following are current listings of our programs, classes, and services at Mercy H.E.A.R.T. Center. Please call the H.E.A.R.T. Center to verify dates and times at (231) 672-3648, as each service and price listed is subject to change at any time. We will be happy to assist you in signing up for a class or scheduling an appointment. Qualifications must be met for some programs offered.

Class / Program Description of Program Dates and Times Cost
Heart Healthy Diets Education on low fat, low cholesterol, and low sodium diets. To discuss dietary changes recommended by your doctor. Given by a registered dietitian. Mondays: 9:00 AM, 1:00 PM Wednesdays: 10:00 AM Tuesdays: Classes last 1 hour. One class free of charge for current Phase II Cardiac and Pulmonary patients.
Healthy Eating Discusses restaurant eating, grocery shopping, and meal planning. Fourth Tuesday of every month 4:00 – 5:00 PM

$20 per person. Free of charge for current H.E.A.R.T. Center clients.

Bariatric Surgical Weight Management Program For patients who have had gastric bypass surgery. Clients will receive dietary and exercise instruction from a dietitian and exercise physiologist. Use of gym facilities is included. Call for details. Patients can begin before or after surgery, per physician recommendations. Variable days and times. Call for price information.
Center for Weight Management: Medical Weight Management Program A 26-week program with use of gym facilities and including assessments by an exercise physiologist, dietitian, and nurse practitioner. Weekly educational sessions held. Continuing follow up appointments by Nurse Practitioner weekly. Call for information to enroll. Call for price information.
Weight Loss Counseling Education on dieting and weight loss. Call to arrange an appointment. Offered on an individual basis. Up to 1-hour sessions. $20 per 15 minutes.
Mercy on the Move: Non-Medical Weight Management Program A 12-week program focused on health promotion and weight management through nutritional counseling, a personalized physical activity program, and education to help you achieve a healthier lifestyle. Use of exercise equipment included. New classes to be offered soon. $25 per week $20 for Mercy Health Partner employees
Diabetic Education Class Education on diabetes management, medications, diet, and exercise. Given by a Certified Diabetic Educator. First Tuesday of every month. 2:00 – 4:00 PM $42. MHP employees eligible for discount.
Pulmonary Education Classes tailored to the needs of those with lung disease. Various topics covered. First and Third Wednesday of every month. 10:00 AM, 2:00 PM, 5:00 PM Lasts 1 Hour. Included for Pulmonary Rehab Patients.
Cardiac Education Offered during hospitalization and during Cardiac Rehab Classes on an individual basis. Risk factor modification discussed in order to maximize cardiac health. Will vary on individual basis. At time of hospitalization. Included for Cardiac Rehab Patients.
Lipid Profiles and Glucose Screenings A blood draw to obtain Total Cholesterol, LDL, HDL, Triglycerides, and Glucose values. Open to community. Results sent to your home in about 1 week. 12 hour fast required prior to testing. Second Tuesday of every month. Walk-ins between 7:00 AM and 12:00 PM. $15 for a Lipid Profile $6 for a Glucose Test
Cardiac Rehabilitation, Phase I Begins in the hospital after the patient has experienced heart problems. Provides exercise, counseling, and education critical for improving cardiovascular health and quickening your recovery. A Phase I Therapist will meet you in the hospital. A referral is made for this appointment by your surgeon or cardiologist. Generally covered by insurance companies.
Cardiac Rehabilitation, Phase II Begins in the weeks after your heart illness.  It is a monitored exercise program to help strengthen your heart and body, and quicken your recovery.  Patients work with a therapist who counsels them on risk factor and lifestyle changes to improve your heart health. Initial assessment made by appointment.  Referral is necessary from your physician, cardiologist, or surgeon. 

One hour rehab classes held on Mondays, Wednesdays, and Fridays at 7, 8, 9,11, 2,and 3 PM.

Covered by most insurance companies.  Amount of coverage will differ.  Call your insurance company to inquire about your benefits before beginning.
Cardiac Rehabilitation, Phase III A medically supervised exercise program in which individuals learn to self-manage their overall cardiovascular health.  Designed for people with known heart disease, known risk factors fro heart disease, or for those simply wishing to improve their fitness. Initial assessment made by appointment.  Referral necessary from your doctor.  One hour rehab classes at 7, 8, 9, 10,11, 2, 4, and 5:15 PM. $5.50 for each 1 hour class.  Generally not covered by insurance. Call your insurance company to determine.
Cardiac Rehabilitation, Phase IV Designed for patients who have shown medical stability and wish to move on to a more independent exercise program.  Certain eligibility requirements must be met.  Exercise is done independently with self-monitoring and recording of your own progress. Any time during normal business hours Monday through Friday. $45 per month.
Pulmonary Rehabilitation, Phase I Begins in the hospital after the patient has experienced lung or breathing problems.  Provides exercise, counseling, and education critical for maintaining and improving pulmonary functioning. A Phase I Therapist will meet with you in the hospital.  Referral is necessary for this appointment by your physician. Generally covered by insurance company
Pulmonary Rehabilitation, Phase II Begins in the weeks after your pulmonary illness.  Consists of a monitored exercise program, respiratory care instruction, smoking cessation, nutritional counseling, psychosocial support, oxygen therapy, and education. Initial assessment by appointment and scheduled by your physician or Phase I Therapist.  One hour Pulmonary Rehab classes at 7, 8, 9, 10,1, 3, 4, and 5 PM. Coverage by most insurance companies.  Amount of coverage will differ.  Call your insurance to inquire about your benefits.
Pulmonary Rehabilitation, Phase III A medically monitored exercise and risk factor modification program similar to Pulmonary Phase II, but geared toward those with existing disease processes.  No new diagnosis. An initial assessment must be scheduled if you have not attended Phase II Pulmonary Rehab.  Rehab classes at 7, 8, 9, 10, 1, 3, 4, and 5 PM.

Classes last 1 hour.
$5.50 per 1-hour session.  Generally not covered by insurance.  Call your insurance company to determine.
Congestive Heart Failure Support Group Discussions of various heart-related topics.  Group support aids in helping those living with CHF to cope with the illness. Third or Fourth Tuesday of every month.  Call to determine dates and times. No cost for patients of CHF Clinic.
Heart Failure Clinic

A clinic offered to those with Congestive Heart Failure and prescribed by your physician.  Health care team works with the patient to manage symptoms through managing medications, risk factor modification, and providing education and support. Appointments made each week for 8 to 12 weeks.  Need referral from physician.  Call for details. Generally covered by your insurance company.
Coumadin Management For those with medical diagnoses which warrant a patient to begin coumadin therapy.  Managed by a nurse practitioner. Blood draws done routinely to keep you in a therapeutic range. Generally covered by your insurance company.
