The need for a Diabetes Management Clinic in Muskegon began as a vision of Dr. Hernan Guiang, an experienced leader in the treatment of difficult-to-manage diabetes and endocrine patients.

The diabetes management team includes a physician, medical assistant, nurse practitioner/certified diabetes educator, and other certified diabetes educators. This multidisciplinary team works closely with each patient and his or her primary care provider to seek a significant drop in diabetes-related complications.

New Patients

At MHP’s Diabetes Management Clinic, we know how important it is to get all your diabetes testing and needed procedures as quickly and easily as possible. During your first appointment, a medical assistant and physician, who are diabetes specialists, will assess you.

You will be trained on blood sugar monitoring and managing diabetes management.
 
Working with your primary care physician, our diabetes specialists will make the appropriate referrals for diabetes diet education, and care for diabetes complicates as needed.

To be seen at the clinic, you will need a written referral from your primary care physician. If you do not have a primary care physician, you may call the physician referral line at 231-672-4500.

Monday through Thursday from 8:00 a.m. to 1:30 p.m.

Appointments

Referrals: Patient must be seen by their primary care physician and obtain a referral prior to being seen in the Diabetes Management Clinic. You may fax your referral to the Diabetes Management Clinic at 231-672-4223 or by calling 231-672-4222

Existing Patient:  Please call the Diabetes Management Clinic directly at (231) 672-4222, if you are unable to keep your appointment. Please call us as far in advance as possible so we may use that time to see another patient in need of care.

Diabetes Education

The Diabetes Education staff provides a comprehensive array of programs and services that are focused on the healthcare needs of patients living with diabetes. Education sessions are taught by American Diabetes Association certified educators. For more information, please call the Diabetes Management Clinic at 231-672-3648.
